Introduction: The Fournier’s disease is a gangrene of soft tissue from the perineal of genital area. Caused by the synergic action of aerobic and anaerobic microorganisms, with evolution may be lethal and it appearance is a frequent in the general surgery services. This study revises data from clinic charge diseases of base, origin, treatment and results. Patients and methods: This study is observational, descriptive and retrospective of 46 patients with Fournier’s disease, treaded in the 2º CCQca., HC, FCM-UNA, and Hosp.Nac.Itauguà, in the period of time of 10 years. From march of 1995 to fever of 2005. Results: 37(80,4%) patients were male and 9(19,6%) were women. The average age was 62 years (from 35 to 85 years old). The etiology was anal abscess in 35(76,2%) patients, urology pathologies in 10(21,7%) patients and in 1(2,1%) patient piodermitis. The most common sin toms were fever and pain ( 36 and 32 respectively). The pathology of base most frequent was de Diabetes Mellitus in 28 patients , were Arthritis Reumatoidea in 1 patient, chronic elitism 1 patient, Tuberculosis in 1 patient. The treatment was surgery with great debridement in more than one opportunity in all of cases, citostomy in 2 patients and colostomy in 22 patients, the antibioticoterapy was cefotaxima of ciprofloxacin with metronidazol. 18 patient’s ingresses to ICU, 7 died for septic shock. Time of light was 45 days (range 4-201 days). Conclusions: The clinical manifestations allayed us to diagnosis. The treatment must be quick and aggressive with large dissections. More than the half of cases the origin was anal and perianal infections, and the frequent disease associated and predispose was the Diabetes Mellitus.
Introducción: La enfermedad de Fournier es una gangrena de los tejidos blandos, localizada en las regiones perineal o genital, causada por acción sinergística de microorganismos aerobios y anaerobios, cuya evolución es potencialmente letal y de aparición relativamente frecuente en los servicios de Cirugía General, en nuestro medio. Objetivos: En el presente trabajo se revisa datos sobre el cuadro clínico, enfermedades concomitantes, sitio de origen, tratamiento y resultados. Pacientes y métodos: Estudio observacional, descriptivo y retrospectivo de 46 enfermos tratados en la II Cátedra de Clínica Quirúrgica, Hospital de Clínicas, FCM - UNA y el Departamento de Cirugía del Hospital Nacional de Itauguá, en un periodo de 10 años ( marzo 1995 a febrero 2005). Resultados: fueron varones 37 (80,4%) y 9 mujeres (19,6%). La edad promedio fue de 62 años (extremos entre 33 a 85 años). La etiología fue: absceso anal 35 pacientes (76,2%), afecciones urológicas 10 (21,7%) y pío dermitis 1(2,1%). Los síntomas más comunes fueron fiebre y dolor (36 y 32 pacientes respectivamente). La enfermedad de base más frecuente fue la Diabetes Mellitus (28 pacientes); otros fueron artritis reumatoide, Etilismo y TBC. El tratamiento quirúrgico consistió en desbridamientos amplios, en más de una oportunidad. La citostomía se realizó en 2 pacientes y colostomía en asa en 22 pacientes. La antibióticoterapia implementada fue cefotaxima o ciprofloxacina, acompañada de metronidazol. Ingresaron a UCI 18 pacientes, de los cuales fallecieron 7 por shock séptico. El tiempo de hospitalización tuvo un promedio de 45 días (rango de 4 a 201 días). Conclusión: El cuadro clínico permite el diagnóstico fácil. El tratamiento debe ser agresivo, precoz, con resecciones amplias y reiteradas. Más de la mitad se originan en infecciones anales y la diabetes mellitus es la enfermedad condicionante más frecuente.
